Client - Doula Agreement

Role of the Doula

 A doula is a trained, non-medical support professional who provides continuous physical, emotional, and informational support during pregnancy, labor/birth, and/or postpartum.  

Scope of Practice

The Doula is a non-medical support professional. The Client understands and agrees that:

  • The Doula does not perform clinical tasks, such as blood pressure checks, fetal heart monitoring, vaginal exams, or catching the baby.
  • The Doula does not diagnose medical conditions or give medical advice.
  • The Doula does not make decisions for the Client but supports the Client in advocating for their own preferences.
  • The Doula works collaboratively with the Client's medical care providers (Doctor, Midwife, Nurses) and does not interfere with their medical care.

The Doula agrees to provide the following services:

The Doula agrees to provide the following services:

  • One initial visit. 
  • Up to eight additional visits that may be provided in any combination of prenatal and postpartum visits. 
  • Support during labor and delivery (including labor and delivery resulting in a stillbirth), abortion or miscarriage. 
  • Up to two extended three-hour postpartum visits after the end of a pregnancy. 

Insurance Billing

All insurance billing occurs only after services have been rendered.

Termination

Either party may terminate this agreement at any time for any reason by providing written or verbal notice to the other party. 

Liability and Indemnification

The Client hereby releases the Doula from any liability for any injury or damages that may occur to the Client or infant during the birth process, provided that the Doula has acted within the scope of their training and this agreement. The Client understands that the Doula is not responsible for medical outcomes. 

Privacy Policy

Client information is confidential and is not shared with any 3rd party (other than for billing purposes).
