Our Mission

Our mission is to improve health outcomes for families and transition-age youth by removing barriers to care, simplifying health system navigation, and coordinating timely access to medical, behavioral, developmental, and social support services through person-centered telehealth and in-person guidance.

Providing Personalized Care and Support

Our  purpose is to provide structured, relationship-based health care navigation services that help families and young adults: 

  • understand their benefits and care options,
  • access needed providers and services, 
  • follow through on treatment plans,
  • overcome logistical barriers,
  • improve communication with providers, and
  • transition more successfully between systems of care.

Beyond CCS Medical Conditions

Transition Ages 18-21

Parents and young adults with complex health needs often face significant barriers when trying to navigate the health care system. These barriers include difficulty understanding insurance coverage, delays in specialty referrals, lack of follow-up after hospital visits, poor coordination among providers, transportation challenges, and confusion when transitioning from pediatric to adult care.
